    $525 Early Bird price, through January 31
    $575 Full price, February 1-April 30

    $100 non-refundable deposit required to complete registration. You can pay online or by check.

    There will be opportunities for fundraising and to make payments.

  • I, the parent/legal guardian, grant permission for my child * to participate in this parish event that requires transportation to a location away from the parish. This activity will take place under the guidance and direction of parish employees or volunteers from St. Mary of the Lake, Mary, Mother of the Church, St. Joseph, St. Edward's, Risen Savior, St. Elizabeth Ann Seaton, nativity of Mary and other parishes in the Archdiocese of St. Paul and Minneapolis, as well as staff and volunteers at Trinity Woods Camp and Retreat Center.

  • I understand that as a parent and/or leagl guardian, I remain legally responsible for any personal actions taken by my minor child, named above ('participant'). I understand that my child is required to comply with the Code of Conduct provided by the parish whil eparticipating in the event. I understand and agree that if my child violates the Code of Conduct he/she may have to be transported home at my expense.

    I agree on behalf of myself, my child named above, or hour heirs, and assigns, to hold harmless and defend St. Mary of the Lake, Mary, Mother of the Church,  Risen Savior, and all other participating parishes, their officers, directors, employees and agents, and the Archdiocese of St. Paul and Minneapolis, its employees and agents, chaprones, or represetatives associated with the event and and activities (hereinafter 'Releasees'), from any claims, including, but not limited to all claims relating to communicable disease, ariing from or in connection with my child attending the event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith and I agree to compensate Releasees for reasonable attorneys fees and expenses which may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the neglgencee of releasees.

    By signing this electronic form I also hereby warrent that to the best of my knowledge, my child is in goodhealth and I assume all responsibility for the health of my child.

  • Code of Conduct

    Read through this with your child
  • The following are a few rules that all participants are expected to follow while participating and representing St. Mary of the Lake, Mary, Mother of the Church, Risen Savior, St. Joseph, St. Edwards, St. Elizabeth Ann Seaton, Nativity of Mary and other participating parishes.

    Treat all other persons with respect and not cause any intentional harm (physically, emotionally, or spiritually) to any person in anyway.

    Respect the property of others, including all program facilities and property. Follow all appropriate instructions of all personnel aiding in this event, including, but not limited to, chaperones, support staff, transportation personnel and administration.

    Not have in my possession any tobacco, alcohol or any controlled illegal substance.

    Does not participate in ‘bullying’ as defined below.
    Bullying is unwanted, aggressive behavior among school-aged children that involves a real or perceived power imbalance. The behavior is repeated, or has the potential to be repeated, over time. Bullying can cause serious lasting effects for both parties involved.
    To be considered bullying, the behavior must be aggressive and include:
    An Imbalance of Power: Kids who bully use their power – such as physical strength, access to embarrassing information, or popularity – to control or harm others. Power imbalances can change over time and in different situations, even if they involve the same people.
    Three types of bullying: verbal, social, and physical
    Verbal bullying is saying or writing mean things. Verbal bullying includes:

    • Teasing
    • Name-calling
    • In appropriate sexual comments
    • Taunting
    • Threatening to cause harm

    Social bullying, sometimes referred to as relational bullying, involves hurting someone’s reputation or relationships. Social bullying includes:

    • Telling other children not to be friends with someone
    • Spreading rumors about someone
    • Embarrassing someone in pubic or online
    • Speaking behind their back in earshot of the victim
    • Ignoring them on purpose
    • Excluding others intentionally, for no other reason than to exclude them
    • Leaving someone out on purpose: both directly and indirectly
    • Telling other children to exclude a particular person from games or other group activities. Walking away or ignoring others when they attempt to join the group
    • Social reputation (e.g. writing rumors or insults about someone on a bathroom wall or an insult or slur on someone’s personal items/social media)

    Physical bullying involves hurting a person’s body or possessions. Physical bullying includes:

    • Hitting/kicking/pinching
    • Spitting
    • Tripping/pushing
    • Taking or breaking someone’s things
    • Making mean or rude hand gestures

    Actions and Consequences:
    Any participant who fails to follow this Code of Conduct will be addressed in order to uphold the morale and environment of our ministry. Minor violations to the Code of Conduct will first be addressed with a verbal warning. In the case that the violation is repeated, parents will be notified and asked to review the Code of Conduct with their child. If the violation occurs again it will be considered a serious violation. Serious violations to the Code of Conduct will result in a conversation with a parent to discuss a proper way to move forward, or being sent home from camp at the parents expense.

  • AUTHORIZATION, CONSENT AND RELEASE FOR USE OF VISUAL LIKENESSES AND ORIGINAL WORKS OF MINORS

    This form allows you, the parent or guardian, to identify if images of your child and their original works may be used for purposes of print, online, social media communication and promotion.
  • I am the parent or legal guardian of * ("My Child").

  • I grant the following rights to St. Mary of the Lake Catholic Church, other participating parishes, and the Archdiocese of Saint Paul and Minneapolis:

    1.     The right to use all photographs, pictures, portraits, vocal sounds, appearances/likenesses, video and performances (hereinafter collectively known as “image”) of My Child in the possession of St. Mary of the Lake Catholic Church;

    2.     The right to use, reproduce, publish, exhibit, distribute, and transmit the image of My Child individually or in conjunction with other images or printed matter in the production of brochures, slides, motion pictures, broadcasts (radio, television, and other social and digital media), audio or video files, recordings, still photography, CD-Rom and any other manner of media now known or later developed;

    3.     The right to use, reproduce, publish, exhibit, distribute, and transmit the image of My Child individually or in conjunction with other images or printed matter St. Mary of the Lake Catholic Church and the Archdiocese of Saint Paul and Minneapolis’s Internet websites. No home address or phone number will be published;

    4.     The right to record, reproduce, amplify, edit, and simulate My Child’s image and all sound effects produced;

    5.     The right to copyright, in the name of St. Mary of the Lake Catholic Church and the Archdiocese of Saint Paul and Minneapolis, works that contain the image of My Child;

    6.     The right to use and publish for general communications, advertising, commercial or publicity purposes, or for any other lawful purpose whatsoever My Child’s original work; and

    7.     The right to assign the above-mentioned rights to third parties without notice to me.

    I understand that the video files, still photos, or other media incorporating the image of My Child will become the property of St. Mary of the Lake Catholic Church. I hereby waive the right to inspect or approve the image or any finished materials that incorporate the image.

    I understand and agree that no compensation will be provided, now or in the future, in connection with the use of My Child’s image or My Child’s original work.

    I hereby release, discharge, and agree to indemnify and hold harmless St. Mary of the Lake Catholic Church , the Archdiocese of Saint Paul and Minneapolis, and their agents, employees and assigns from any and all claims, demands, right, and causes of action of whatever kind that I or My Child have or may have or may arise by reason of this authorization and from the use of My Child’s image and original work, including but not limited to, all claims for libel and invasion of privacy.

    This consent regarding My Child’s likeness and original work is valid until such time as I choose to rescind this authorization and consent. If I choose to rescind this authorization and consent, I agree that I will inform St. Mary of the Lake Catholic Church in writing and that my rescission will not take effect until it is received by St. Mary of the Lake Catholic Church. I understand and acknowledge that it may not be possible to recall any work or photos that have been published prior to receipt of my written rescission.
